How to Use This Checklist

  • Note which symptoms you are experiencing and how often
  • Rate each one for how much it affects your daily life
  • Track for a few weeks to reveal patterns and triggers
  • Bring your notes to your provider or to our conversation

Physical

  • Hot flashes or night sweats
  • Disrupted or lighter sleep
  • Fatigue or lower energy
  • Weight changes, especially around the middle
  • Joint aches or stiffness
  • Headaches
  • Heart palpitations

Cognitive and Emotional

  • Brain fog or trouble concentrating
  • Memory lapses
  • Mood swings or irritability
  • Anxiety or a shorter fuse
  • Low mood or loss of motivation
  • Feeling not quite like yourself

Whole-Body and Recovery

  • Slower recovery from exercise
  • Lower stress tolerance

What to Do With Your Answers

A pattern of symptoms is information, not a verdict. It helps you and your provider decide what to explore, and it helps me understand how to support you, whether through coaching, a restorative journey, or both. Symptoms rarely exist in isolation, and reading them together is the first step toward a plan that fits your body rather than a template.
